Collections Check List
To Submit a Collection - Please Use the Following Checklist
Collection Amounts Must Be $100 or More to Submit

Must Have
  • NAME OF TENANT(S)
  • LAST KNOWN ADDRESS
  • SOCIAL SECURITY NUMBER(S)
  • ACCURATE BALANCE
  • ITEMIZED STATEMENT - Detail how the balance was arrived at. Damages added to every balance MUST be documented with pictures and/or video. You do not have to submit the pictures or video, however, you must keep these in your records if the case goes to court. Please make sure that damages do not include "Normal Wear & Tear" such as if the tenant has been in the unit for over 3 years you cannot put carpet replacement as damages. Same with painting the unit.
  • SIGNED CONTRACT BETWEEN LANDLORD AND TENANT (Rental Agreement, Lease Agreement, Addendums etc...)
  • DATE OF LAST CHARGE (Required in order to report to Credit Bureau)
  • COPY OF BLANK RENTAL APPLICATION
  • RENTAL APPLICATION
    (Filled out by tenant)
  • MOVE IN & MOVE OUT DATES
